If not already pre-booked, notify Concord Dental Lab of your intention to initiate a pre-booking for a new case.
- Patient’s Full Name.
- Specific Procedure(s) to be performed.
- Scheduled Appointment Date and Delivery Time (date and time case needs to be delivered).
- Anticipated Pick-Up Date (date when case materials will be ready for pickup by Concord Dental Lab, if applicable).
- Clarify whether the payment responsibility is with the doctor’s office or the patient. This information will be recorded by Concord Dental Lab for billing purposes.
To accurately process and fulfill your case, please communicate all specific instructions, objectives and design considerations, and any additional charges that may apply. Concord Dental Lab will document this information as follows:
- Dentist Instructions
- Case Objectives and Design Considerations
- Additional Charges (e.g., rush orders, special materials and who is responsible for these charges).
- Document Pre-Booking case number from Concord Dental Lab.
- Confirmation – Upon providing all necessary information, Concord Dental Lab will finalize the pre-booking and ensure you receive confirmation of the pre-booked case, including any reference numbers or additional instructions.
