and conditions.

Payment terms

Each case will be returned with an invoice. A statement will be provided at the end of each month. All statements must be paid in full by the 15th of the month in which the statement was prepared. Any amounts not paid by the last business day of such month will incur a 2% finance charge per month. The account will be automatically placed on C.O.D. for any amounts not paid after 90 days. If an account becomes more than 90 days past due – the balance due will be placed on the card on file.

We accept Cash, Check and All Major Credit Cards. All cases and items sent remain the property of Concord Dental Laboratory, Inc. until the client’s account is paid in full. A minimum of $50.00 will be charged for returned checks.

All disputes shall be governed in all respects by Massachusetts law and client agrees to submit to the exclusive jurisdiction of, and venue in the County of Middlesex, State of Massachusetts in any dispute, with the prevailing party to recover attorney’s fees, court costs and other expenses, including actual expert witness fees, if any, in addition to any other relief to which prevailing party may be entitled.A credit card is required on file, but will not be used to make payment on the account without notice.

Refund policy

The cost of fabricating Concord Dental Laboratory dental restorations cannot be refunded. A credit may be issued by Concord Dental Lab directly to a customer’s account when cases do not meet expected standards for quality, function and aesthetics. Services fees and parts fees are not refundable unless the product can be returned to the retailer for credit. Refunds on Implant components are pursuant to the individual manufacturer return and warranty policies.

Remake policies

As a Lab, we view ourselves as a member of your dental team, and we intend to do our part in being a good team member. All single unit* remakes will be at no charge except under the following circumstances:

  1. Concord Dental Lab inquired about the die, margin or impression. However, the customer approved and requested completion of the case.
  2. Concord Dental requested a try-in, but the customer declined and asked for a completed case.
  3. A try-in was completed and approved by the office.
  4. The teeth are re-prepared.
  5. There is a shade change/correction different from the original request.
  6. A practice has risen into our top 5% for remakes and has been identified with a chronic remake problem with numerous no lab-error remakes**.

No Lab Error Remake Reasons include:

  1. Cases that are re-prepped at attempted insert of crown/bridge
  2. Crown provided fits master die/model but does not fit mouth indicating poor original impression or temporary-driven drift
  3. Final restorative shade or product change different from the original request.
  4. Treatment planning was incomplete or not followed on large multi-step cases.

*All Multi-unit case remakes are subject to a manager review and are only at no charge when found to be a lab error.
**If any of the no-lab error events listed above are the sole reason for remake with any client, the remake may be shared by billing material cost only, or no charge at the discretion of CDL management.

denture & appliance

Remake policies

If the original models(s) and appliance/device/restoration(s) are returned within 60 days from the original invoice date, CDL will issue a credit (partial or in full) to the dentist’s account. Concord Dental Laboratory will remake any case at no charge if received within 30 days of the invoice date except under these circumstances:

  1. Appliance, or denture fits the master die/model.
  2. Concord Dental Laboratory inquired about the impression and/or model however, the dentist approved and requested completion of the case.
  3. Concord Dental Laboratory requested a try-in but the dentist declined and asked to complete the case.
  4. Denture (full or partial) set-up was approved and the case was processed to completion.
  5. Tooth/teeth were re-prepared.
  6. A new impression was supplied.
  7. Final restorative shade or product change different from the original request.
  8. The case was canceled after fabrication was initiated. Fabrication starts the day Concord Dental Lab receives the case; the case will remain billed at full cost.

Return policy

If the original model and dental restoration are returned within 60 days from the original invoice date, CDL will issue a 100% credit to the customer’s CDL account. In order to receive a credit for work that is returned, Concord Dental must be given the opportunity to fix or replace the original restoration. If cases are returned without the opportunity to fix or remake, alloy charges will be refunded but the porcelain and labor charges will remain on the account.

Limited warranty

Concord Dental Laboratory’s warranty covers the repair or replacement of products that are inserted by a licensed dentist, however excludes:

  1. Cash refund of the product.
  2. Any costs incurred for removal or insertion.
  3. Any incidentals or consequential damages including inconvenience, lost wages, chair-time, or pain and suffering.
  4. Any repairs resulting from accident, neglect, abuse, failure of supporting tooth or tissue structure, improper adjustments or poor dental hygiene.
  5. Any multiple unit fixed prosthesis or any removable prosthetic that has not been appropriately verified or fitted prior to final processing.
  6. Any repairs, relines, temporaries, implants, immediate dentures or partials, and appliances partially or completely fabricated by another laboratory.
