Rebates are calculated based on the completion date of each case, not the arrival date or requested return date. Completion dates are determined solely at the discretion of Concord Dental Lab (CDL). Accounts with past-due balances of 60 days or more will not earn rebates during any month such a balance is outstanding. To qualify for the program, any group of dentists combining their volume must be billed as a single entity with a consolidated monthly statement. Participation may be restricted for practices with pre-negotiated discounts or custom pricing agreements, at CDL’s sole discretion. Rebates are issued in the form of statement credits.
